You must be a Warren County resident to schedule an appointment.  All appointments will be in-person.  

Please note: Our office is opened Monday through Thursday and we are closed on Friday.

Our address is 645 Oak Street, Lebanon, Ohio, 45036. The HEAP Department entrance is located at the front of the building. Designated parking for visitors is located at the front of the parking lot, off Oak St.. 

Please note: all appointments will be in-person for the Percentage of Income Payment Plan known as PIPP. City of Lebanon customers are not eligible for PIPP.

The Summer Crisis Program will provide income-eligible households with a one-time benefit to assist with an electric bill and/or purchase an air conditioner and/or fan:

To qualify for the Summer Crisis Program, you must meet at least one eligibility criteria listed below:

● A household whose electric has been disconnected, has a disconnect notice or new service is being established.

● A household member who is 60 years of age or older.

● A household member with a documented medical condition.

City of Lebanon customers are not eligible for the Percentage of Income Payment Plan (PIPP). Co-pays must be paid prior to receiving a benefit and documentation verifying the receipt of the co-payment must be obtained prior to the Summer Crisis Program application approval or verification of enrollment in a payment plan.

You may upload your documents prior to your appointment, by clicking on the Document upload button to the right of this screen.   If you are unable to use the on-line system to submit your application documents, you will need to bring all required documentation to your scheduled appointment in paper form.

Please note: During the Summer Crisis Program, there are no holds placed on electric utility accounts when making an appointment. Funding for the Summer Crisis Program is limited, and so it is strongly encouraged that you arrive for your appointment with all required documentation as failure to do so may result in a disconnection of service and no financial assistance.

Failure to submit all the required documentation will delay your assistance. If you are unable to upload the required documentation to the system, please provide all documents in paper form at your appointment. No exceptions.

  • Income must be last 30-days from appointment date:

    • Pay stubs must be gross income, must include name and employer name & address 

    • Social Security / SSI / Pension (when applicable: provide proof of disability doc)

    • Court Ordered Child Support Payment History

    • Worker's Compensation

    • TANF / OWF Benefits

    • Unemployment

    • Utility Reimbursement

    • Self-Employment - (ex. landscapers, home childcare providers, odd jobs, etc.)  Proof of the past 12 months.  A Self-Employment form and prior year tax return, IRS tax transcript or non-filing status.  You can download a Self-Employment form from our website at:  www.wccsi.org     

    • Seasonal Employees - (ex. Landscapers, school employees, etc.) - paystubs, printout or Seasonal Employment Verification form for the past 12 months. Pick up form prior to your appointment or go to our website at: www.wccsi.org 

    • Zero Income - Required: If someone has given you money for rent, bills (including your rent/mortgage, food, utilities, medical and any assistance received). A letter from the gift giver with the amounts given and frequency and their name, address & phone number. Must provide documentation if behind on bills

    • Most Current Utility bill in your name/Invoice from bulk fuel/propane

    •       Citizenship
      • Proof of Citizenship (birth certificate, social security cards, U. S. Passport, etc.)

      • Renters: Proof of Residency

      • Copy of most current lease or statement from the landlord

      • Metropolitan / HUD / Section 8 housing worksheets- (most current year)

        • HUD participants must have landlord information

      • Food Stamps:

      • Food Stamps -most current letter or printout from agency
